Your First Visit

For your first visit please arrive 15 minutes early in order to fill out paperwork.
You will need your Doctor’s prescription (Rx) for physical therapy. The prescription must include your doctor’s diagnosis (Dx). Prescriptions can be faxed directly from the physicians office to our office, our fax number is (646) 497-1459.
Bring your insurance card(s).
You will need a photo ID for security in the lobby.
Dress comfortably. It is not necessary to bring sweatpants or work-out clothes to your first session, unless your Physical Therapist has asked you to do so.
Insurance Plans

Physical Therapy & Feldenkrais NYC is an authorized Medicare provider. We are “out-of-network” with all other insurance carriers. To determine the eligibility and benefits of your particular plan call us at (646) 497-1480.
When you call for the first time, be ready to provide your name, date-of-birth, insurance carrier name and ID#. This allows us to verify your benefits with your provider. We’ll call you back the same day, and give you the details of your benefits for physical therapy, including:
- Deductible and balance
- Percentage paid
- The number of visits you are allowed per year or per condition
